This is a first amendment to the contract for the Cleaning and Modernization of Elevators at the Holley Court, the Avenue, and the Oak Park River Forest High School Parking Structures, to adjust the scope of work for the urgent repair of the east elevator at Holley Court Garage.

Fiscal Impact
The original scope of work in the independent contractor agreement included mechanics modernization to the West Elevator at the Holley Court Garage in the amount of $59,780. This item will be removed from the scope of work and $59,780 will be added to the Parking Fund 2019 capital budget to be completed under a new agreement next year.
The urgent repair of the East elevator will be added to the scope of work in the independent contractor agreement for $52,985. This is $6,795 less than the item being removed.
The contingency amount in the independent contractor agreement will be increased by $6,795, from $4,904 to $11,699. As such, the independent contractor agreement not to exceed amount of $170,000 will remain unchanged.
Background
The Village board approved an independent contractor agreement with South West Industries, Inc. d/b/a Anderson Elevator Co. at its September 4, 2018 regular meeting. The scope of work included shaft & pit cleaning of all Village parking garage elevators, cab body maintenance at the east elevator at Holley Court Garage, interior cab cleaning at all elevators at Holley Court Garage, and mechanics modernization at the east and west elevators at Holley Court Garage.
On September 18, 2018, the east elevator at Holley Court Garage failed its annual Category Test Inspection. The elevator has been shut off until repairs and re-inspection can take place.
The Village received a quote from South West Industries, Inc. d/b/a Anderson Elevator Co. to replace the failed underground cylinder for $52,985. Due to the urgent need to replace the cylinder and return the elevator into service, the Village is amending the scope of the recently passed independent contractor agreement with South West Industries, Inc. d/b/a Anderson Elevator Co. The revised scope will remove the mechanics modernization to the west elevator and replace it with the furnishing, installing, and testing of a new cylinder for the east elevator. The original agreement’s not to exceed amount of $170,000 will be unchanged.
