Regular Village Board meetings are typically held at 7:00 p.m., the first three Tuesdays of each month in Council Chambers of Village Hall (room 201), 123 Madison St. When a Regular Meeting falls on a holiday, the meeting typically is held the following night. The Health Department has provided Family Case Management Services since the program began 20+ years ago.  The Health Department is recommending the Village Board approve this motion for the Health Department to apply for the Maternal and Infant Health Program’s Family Case Management Grant, with a potential grant revenue of $31,478 for the period of July 1, 2016 to June 30, 2017.

Fiscal Impact

The Family Case Management program is managed by a Registered Nurse working within the Health Department.  The Family Case Management program will be a 0.5 FTE position.

 

Background

The Health Department has performed Family Case Management services since the program began 20+ years ago, with the exception of the time period of November 2014-present; where the Village has been without a Public Health Nurse on staff.

 

Services under this grant will be performed by one Registered Nurse whose office will be located at the Health Department in Village Hall. Through a contractual arrangement with CEDA WIC, the nurse will coordinate case-finding with WIC staff at their local office located at 6026 Roosevelt Road in Oak Park.   The Family Case Management program includes case management of infants and children from the state-wide Adverse Pregnancy Outcomes Reporting System (APORS) and medical case management of Medicaid eligible mothers and children.  The goals of service are to 1) decrease infrant mortality and morbidity; 2) improve pregnancy outcomes and 3) reduce the incidence of prematurity and low birth weight.

 

In addition, the nurse will also collaborate with the Collaboration for Early Childhood Care (collab4kids.org) of Oak Park and River Forest. As part of this collaboration the Collaboration has verbally communicated a possibility of providing up to $15,000 to fund the nuse case manager’s salary.  The Collaboration performs outreach to families with children from birth - 5 years to link them to quality, affordable, accessible childcare and pre-school as well as to early intervention services. This partnership will allow the FCM to identify and locate families with infants, and pregnant women, who may be eligible for FCM services.

 

The grant is a “fee for service” grant, where grant monies are awarded based on cases served by the nuse case manager.  In addition to the possible grant revenue of $31,478, the Village will also bill Medicaid for indirect costs associated with managing the Medicaid eligible families through this program.

 

Note that the grant application deadline is September 2, 2016.  If the Village Board does not approve this motion, the Health Department will withdraw the grant application.

 

 

Alternatives

The Village Board can choose to not approve the motion.  If not approved, the grant application (which is due on September 2, 2016) will be withdrawn by the Health Department.
