Regular Village Board meetings are typically held at 7:00 p.m., the first three Tuesdays of each month in Council Chambers of Village Hall (room 201), 123 Madison St. When a Regular Meeting falls on a holiday, the meeting typically is held the following night. The Village Board also meets in special sessions from time to time. However, dates and times of Special Meetings can vary and may change.

This is a one year agreement with the Collaboration for Early Childhood Care and Education.  During the term of this agreement the Village will work with the Collaboration and home visiting partners to establish and implement a coordinated intake and referral system for families who may be eligible for home visiting services.  The Collaboration will compensate the Village $10,000 under this agreement.

The Collaboration for Early Childhood Care and Education to Compensate the Village in the amount of $10,000 to supplement the salary of the Village's Public Health Nursing position.

 

Background

The Village anticipates signing a grant agreement with the Illinois Department of Human Services (DHS) for the Family Case Management program prior to December 31, 2016.  This grant will allow the Village to hire a registered Public Health Nurse to coordinate and manage all aspects of the family case management program, as required under the grant deliverables set forth by DHS.   Under the grant, the registered nurse ensures children under age two who are medically fragile,  low income or living in other very challenging home environments receive visits from the Village’s trained nurse who follows the child to monitor their growth and development.  The Nurse will also provide medical consultation to pregnant mothers through their pregnancy, birth and then up to the child’s second birthday. Equally as important are the referrals and connections to the home visiting programs such as the Collaboration for Early Childhood Care and Education and other partners, ensuring the children and mothers receive care coordination from all available local social service organizations. 

 

During the term of this agreement, the Village will work with the Collaboration for Early Childhood Care and Education and other home visiting partners to establish and implement a coordinated intake and referral system for families who may be eligible for home visiting services.  This partnership will benefit the eligible families, Village, the Collaboration and the partners.  For an in consideration of the services set forth in the agreement, the Collaboration will compensate the Village in the amount of $10,000.

 

Alternatives

The Board could delay action, seek more information or, if not approved, the Village must then determine how to move forward without the external partnership and $10,000 in funding to support the costs associated with hiring a qualified registered public health nurse required for the Family Case Management program.
