Regular Village Board meetings are typically held at 7:00 p.m., the first three Tuesdays of each month in Council Chambers of Village Hall (room 201), 123 Madison St. When a Regular Meeting falls on a holiday, the meeting typically is held the following night. The Health Department is recommending the Village Board approve this inter-governmental agreement with the Illinois Department of Human Services in an amount of up to $31,478 and authorize its execution.  The grant term is from the date of execution to June 30, 2017.

Fiscal Impact

Total funding from this grant is estimated at $31,478.  This is a fee for service grant.  The grant monies will be primarily used to support a portion of the salary of the Public Health Nurse position.

 

Background

The Health Department has performed Family Case Management services since the program began 20+ years ago, with the exception of the time period of November 2014-present; where the Village has been without a Public Health Nurse on staff.

Services under this grant will be performed by one Registered Nurse whose office will be located at the Health Department in Village Hall. Through a contractual arrangement with CEDA WIC, the nurse will coordinate case-finding with WIC staff at their local office located at 6026 Roosevelt Road in Oak Park.   The Family Case Management program includes case management of infants and children from the state-wide Adverse Pregnancy Outcomes Reporting System (APORS) and medical case management of Medicaid eligible mothers and children.  The goals of service are to 1) decrease infrant mortality and morbidity; 2) improve pregnancy outcomes and 3) reduce the incidence of prematurity and low birth weight.

In addition, the nurse will also collaborate with the Collaboration for Early Childhood Care (collab4kids.org) of Oak Park and River Forest. As part of this collaboration the Collaboration will provide $10,000 in annual funding to supplement the nurse case manager’s salary.  The Collaboration performs outreach to families with children from birth - 5 years to link them to quality, affordable, accessible childcare and pre-school as well as to early intervention services. This partnership will allow the FCM to identify and locate families with infants, and pregnant women, who may be eligible for FCM services.

The term on this grant is from the date of execution to June 30, 2017.  Attached you will find a copy of the grant agreement.

 

Alternatives

The Board could delay action and seek more information or, if not approved, the Village would not provide Family Case Management services. 

 

Previous Board Action

On September 6, 2016 the Village Board approved Motion 16-103 directing staff to submit an application with the Illinois Department of Human Services for this grant agreement.
